Abstract: The purpose of this training manual is to guide operation of the Case Management Program for Frail Elders (CMPFE). CMPFE is a coordinated comprehensive system designed to provide Iowa’s elderly with the opportunity to make their own choices regarding long-term care and to receive services in the home and community settings. It gives the Iowa frail elderly population an alternative to a nursing facility and other forms of institutionalized care by coordinating the delivery of needed community services that allow elderly Iowans to remain safely in their own homes. A Case Manager is assigned to monitor each consumer and provide assistance to help the consumer make appropriate choices that best fit his or her needs. This manual starts with background information about the CMPFE history and administration, consumer eligibility, program structure, and general operational standards. It also provides definitions of key terms. Then, it explains the program processes, following the flow of a consumer entering the program from referral through the home visit and evaluation procedures, development of a service plan, ongoing monitoring, and documentation.
